Note # of teams in appropriate space for each gender & age/group.
· Teams are designated BOYS or GIRLS (Do Not Choose Co-ed or BOTH). Girls on Boys team = BOYS team, Boy on Girls team = BOYS team.
· Age of oldest registered player determines age group of play.
· *IF we have sufficient #’s, a separate U15+ (2009-2005) HS age group could be created for competitive balance, in U13+ Age Groups and above.
· ALL participating players are required to be registered (ENYYSA/LIJSL) Rec (Intramural) players, No Player Passes Issued.
· Cost $8.00 per player covers registration/insurance.
· Team registration fee of $75.00 per team, invoiced to each club, Includes scheduling.
· Home Clubs assign, supply referees for each match
