Only Club President or Registrar can request a divisional alignment change for a team.
A separate form MUST be submitted for each team request.
The last day to submit a request is – Wednesday, February 7 by 12 p.m.
Please note the earlier you submit your request the more time it allows us to review, make a decision.
Games committee or office staff may need to contact you or the team coach to discuss your request.
